Sinus Lift
Is a sinus lift a common procedure? Why is is done?
A sinus-lift procedure is a surgical procedure, performed by a specialised dentist, to augment bone mass in the top jaw (maxilla), which increases the likelihood of successful placement of dental implants. Bone from another part of the body, such as the iliac crest, is grafted in the bone (endosseous) below the floor of the maxillary sinus. In the upper jaw the amount of bone is reduced by the presence of the sinus. A number of techniques are used to increase the bone height:
1. Onlay grafts
2. Interpositional (Lefort I) grafts
3. Inlay grafts for nasal floor
4. Sinus-lift and grafting
Surgical technique
Lateral approach is used commonly in the severely resorbed and thin ridge cases. A crestal incision is made with vertical extensions and the lateral aspect of the maxilla is exposed. Then the osteotomy is completed. The lateral wall with the sinus membrane is rotated medially into the sinus. The sinus membrane can fold on itself when reflected medially. Implant sites can be prepared and implants placed at this stage. The medial part of the sinus is grafted first. After the implants have been placed, the remaining lateral part of the sinus defect is grafted. The flaps are relieved and closed primarily. The graft is left for 6-9 months. Implant placements should be delayed if they cannot be properly stabilised, to prevent complications.

Basics:
A major factor for implant success is the quantity and the quality of the bone where the implant needs to be placed. The issue with placing implants In the back of the mouth for the upper jaws is the location of the sinus. Implants need to be placed in a certain amount of bone and they cannot be placed into the sinus cavity. Since the sinus is a membrane, it can be lifted and displaced by bone in a procedure called a sinus lift or sinus augmentation. This does decrease the volume of the sinus cavity but it is rarely ever an issue.

TMJ Wisdom Teeth – How Jaw Problems Can Affect Vocal Ability
Your jaw movement, or lack of movement, has a grim and important affects on your voice. TMJ (temporomandibular joint disorder) is not just now a jaw issue…it’s a vocal problem. TMJ Wisdom Teeth
* First of all, the stiff jaw which accompanies TMJ will keep the soft palate from lifting. Try gritting your teeth while simultaneously attempting to lift the soft palate in a yawn-like sensation. Quite difficult, isn’t it?
* A stiff jaw will inhibit the full opening of vowels, causing a tight throat instead of an open one to constrict singing. * Lack of mobility in the jaw will also cause a singer or speaker to form words and syllables too far back in the mouth, muffling communication and tightening the throat. This is because a tight jaw will cause the base of the tongue to contract, bunch up and pull up on the top of the larynx during articulation.
* A jaw with TMJ can cause a singer to stop truly listening and being able to aim at pitch, especially if there is a ringing in the ears (tinnitus) to conflict with the tones a singer wants to accurately hit. To deal with this, you obviously need to address TMJ solutions, but it’s also important to get back to some pitch practice with a trusted coach. TMJ Wisdom Teeth
When I was a staff-singer at a jingle company in Memphis, I once had a terrible bout with TMJ. It was so bad I had to open my mouth before they turned the mic on so my jaw wouldn’t cause a clicking sound! In my case, it was caused mostly by a new tooth filling -too high- which caused me to grind my teeth to try and close my bite. It was cured by
1. the dentist filing the filling down for a better fit,
2. using a mouth guard to stop my night-time grinding and
3. a short-term course of prescription muscle relaxer to relax my jaw. Whew! Not all causes of TMJ are known. With singers it’s often a case of “which came first, the chicken or the egg?” Tight jaw contributes to tight throat. TMJ Wisdom Teeth
